Dear Doc, I hope you can give me some advice regarding anal intercourse. As the receiving party I find I have very little difficulty with the thickness of my partner. I do, however, have a problem with length. Anything over 7 or 7 1/2 inches seems to hit something deep in my rectum and is painful. On the other hand, I have been able to fully insert a 12 inch long dildo without the same problem, although it did not involve the same kind of thrusting as in intercourse.
Is this a function of normal anatomy, or is there something I can do or some position I should use, to lessen or prevent this? Also, I have an enlarged prostate which is 3 times the normal size. Could this be sufficient to push the penis "off course" so that it hits the wall of the rectum? I am concerned about damage to the rectum, particularly over time, but also because it diminishes a wonderful experience. - ***
Dear ***, It is a matter of anatomy influenced by age no doubt. There is a straight track up from the rectum and along the way the prostate is passed. I expect an enlarged prostate pressing back against the colon can angle your penis so it veers to the pelvis and doesn't make it around the sigmoid angle. The colon angles leftward. I assume your flexible dildo makes the angle easier than a hard member which may also cause pain ramming against the prostate. Slower thrusting may make things less troublesome. I hope that helps explain the situation.

I might add that everyone's anatomy is different. If you are over 50 you should have a colonoscopy especially if the painful intercourse represents a change for you. -Ellar M.D
